How are opens tracked?

The industry standard for tracking opens is determined by a tracking image being displayed in the email client.  When the image is displayed, an open is tracked. 

An open can also be calculated through a clickthrough being recorded, because even if the recipient has images disabled in their email client, when a link is clicked, the open is then assumed.
